I looked around, and it seems like there's an equal split between people who put it as percentages and cases per 100,000. Is it okay if I use both on the test? For example...

Ans: 449 cases per 100,000 (4.49%)

I believe it would be 0.449%...
If they specify which to use or use either in a question, I guess just go with that. I think the way to calculate should be out of 100,000 though.

I think you can actually make it out of 1000 in such cases.

The question will almost always (ideally, should) tell you what to express prevalence and incidence in terms of, whether it is as a percentage, per 100,000, or per 1,000. (i.e. They can be expressed in any of these ways, just make sure you answer the question posed.)
