You group the participants into 2 groups: exposed and non-exposed. Next, you look back to previous records to figure out who has the disease and who doesn't.

What is the name of this study?

Which measures of association do you use in this study?

Actually #2 can't be odds ratio because that's used in Case-control and I basically got everything else right.

Um, Odds Ratio also is used in Retrospective Cohort since Ret. Cohort is very similar to Case Control and is small scale, and you got most of #3 wrong and some of #1 wrong.
Prospective: Looks for outcomes in the future
Retrospective: Comparing past outcomes to figure out a common exposure
