I got permian for 4 but otherwise correct!

8. Sagittal is right but I shouldn't have asked that question
11. No (Namesake is tabullae and you can't see them because its a sagittal cut--> i think you could see them if it was a frontal or transverse cut except I think a frontal is better)
12. Shallow marine is correct but important to note that its WARM water
13. Silurian is correct but this was true during the Devonian as well

2. soft muddy sediment in marine waters
3. none? 
4. radula...........................(jk lophophore)

All good except for 3 they did survive the great Permian extinction. And I know 4 is super easy but was struggling of what else to put. Your turn.

also, I thought Mucrospirifer only lived in the Devonian...?

Wait. How did Mucrospirifer survive the Permian extinction if it is not found after the Devonian?
