Where did you find this? Did you contact Fred Siegel?
Yes, I emailed Fred Siegel (State Director for DC). Their state tournament was theoretically held 3 weeks ago and I was curious as to if DC was going to be sending a team at all.

Yup, Clements, a top 10 team at Nationals, isn't going to nationals after getting second by 4 points. Pretty ridiculous.

If a team from a state with only one bid were to decline their nationals bid, would it simply fall onto the second place team? Is there absolutely no chance of another state (like Texas) being able to regain their second bid?

No, it just goes to second place (third place if applicable).

This article says Central Arkansas Christian High placed 2nd at states. Still haven't found anything about Div B or the Div C champion.

ETA: Div B Champion is LISA Academy West.
