Everything is important. Check out the test exchange for previous Disease Detectives tests (although those were before the rule change).

Is there a main subject this year. If there is, what is it?

Depends on the state. Here in Hawaii, the tests are 2-3, but at nationals, they approach 7-8. In competitive states like California, tests can go up to 9 in difficulty. I have not yet encountered a 10 test thus far.

No main subject, the topic is just general epidemiology.

Like the other person said, it focuses on general epidemiology. I recently went to CyFalls and did Disease there for Div C and it was pretty similar to a Foodborne Illness test from last year. I'd say that its pretty comparable, just bigger sections on random pathogens/diseases, but still the same general topics. Any past years tests would be good practice.
