It is where the more competitive teams are put into aa, while the less competitive ones are put in a. However, almost always, the top a team is in the top 20. Medals are awarded in a and aa, but it doesn't matter in overall ranking.

It is not necessarily large school vs small school. I went to a middle school with 120 kids that was in aa. Also, a and aa are decided by a committee.
