"Clearly Labeled"
In the rules 3f, 3j ii, and 3k iv. it states that [ ] has to be "clearly labeled", but does that mean on the physical roller coaster or the diagram?

not sure - what i would do is label on both just to be safe since either way it wont get counted against you. i label mine on the physical and the diagram for the logs.

It means on the physical roller coaster.
A clear start and finish line, marked perpendicular to the direction of travel of the ball, on the device, so the ES can clearly see when the ball crosses each line.
The start and end of each gap clearly marked (I recommend "Gap 1 Start", "Gap 1 End", "Gap 2 Start", "Gap 2 End") so that the ES can see where the ball is supposed to go, and where they are supposed to measure, and so they can determine whether a gap jump was successful.
For the loop, the ES wants to see what you're claiming as your POI and highest inside point so they know what to measure.
You also have to label them on the design log (rule 4b ii)
